Dr. Mouallem does not participate in any insurance networks, but he will provide statements in support of reimbursement using any out-of-network benefits you may have. Many health insurance plans offer generous out-of-network coverage (often covering 60-80% of the cost), such that high-quality treatment can be affordable and accessible.

To assess how much treatment would cost you out-of-pocket, it is helpful to be familiar with your plan’s benefits:

  • Your plan’s annual deductible – the amount for which you are responsible before insurance shares the cost with you.

  • Your co-insurance rate – the percent contribution that your plan expects you to make after the annual deductible is met. This is usually between 20-40% of the cost.

  • Your annual out-of-pocket maximum – the maximum expenditures out of your own pocket that you are expected to make during the year. This is generally the sum of your annual deductible and the cumulative co-insurance contributions. Once this out-of-pocket maximum contribution has been met, most plans will then reimburse allowed charges in full for the remainder of that calendar year.
